Skillful use of rhyming sounds to bring all the meaning together. I agree with how this sounds, overall, like a song. The second set of stanzas (three and four) would work as a poignant chorus.

When we hear those words, the heart truly believes it is forever. Sadly, it isn’t always the case. This is accurately sad for many of us and you write it beautifully!
